DescriptionThe owners statement provides the date of the statement, lists the parish in which property falls, a description and location of the property, the value of rent received on the property and who received from, and nominates a proxy or a service address. The claimant for the vote has signed the statement and provided a residential address.

The proxy's appointment provides the date of the statement, the name of the appointee, a descritpion and location of the property allowing the entitlement to vote. The appointment is signed by the owner and a residental address is provided.

The proxy's statement provides the date of the statement, the name of the person appointing the proxy and a descritpion and location of the property allowing the entitlement to vote. The proxy has signed the statement and provided a residential address.
